Any patient who has a long term condition such as (but not limited to) Asthma, Diabetes, COPD, Hypertension, CHD etc... will be invited in for an annual appointment with our nursing team. At this review we will perform some tests to monitor the conditions and arrange any follow up as required.

Please make an appointment to see the Practice Nurse who can help you to stop smoking- make your appointment for a day that suits you but please let Reception Staff know it is for smoking cessation advise
