Asthma ClinicFor patients with ongoing conditions, we provide a regular annual review focusing on our patients' priories to support their wellbeing. We aim to have only one or two appointments with our nursing and GP teams to cover all conditions to save patients coming in on multiple occasions. We appropriate, we provide results and care plans as takeaways so our patients feel informed and have a record of their appointments.
We invite patients to attend their review around the month of their birthday.
