Advanced Nurse Practitioner

Claire will be working closely with the GPs to manage acute and chronic conditions. Her duties involve:

- Undertake a comprehensive and sophisticated physical and/or mental health assessment of patients with complex multiple healthcare needs and/or in crisis.

- Interpret the results of multiple different assessments and investigations in order to make a diagnosis, and plan and deliver care.

- Confidently and competently make ethical, evidence based decisions and interventions when faced with complexity and assess and manage the risk associated with these decisions.

- Utilise therapies such as cognitive behavioural therapy when working with patients with mental health conditions either in isolation or associated with a physical long-term condition.

- Prescribe and work with individuals to manage their medicines.

- Work independently but also as part of a multi-disciplinary team and exercise values based leadership.

- Plan and provide skilled and competent care to meet a patient's health and social care needs involving or referring on to other members of the healthcare team as appropriate.

Health Care Assistants are primarily to assist the Practice Nurse.  They can help with health issues such as family planning, healthy living advice, blood pressure checks and dressings. They can also assist the practice nurses with clinics for long-term health conditions such as asthma, COPD or diabetes, minor ailment clinics.  She cannot prescribe medication.

The HCA will also give injections for seasonal flu and pneumonia, and B12 injections.

Practice nurses are qualified and registered nurses. They can help with health issues such as family planning, healthy living advice, blood pressure checks and dressings. The practice nurses run clinics for long-term health conditions such as asthma, COPD or diabetes, minor ailment clinics and they carry out cervical smears.  

She also gives travel vaccinations and Yellow Fever vaccinations; if you are planning to go on holiday please fill out the travel information form before making an appointment to be seen, this is available on this web site. 

The Nurse will also give injections for seasonal flu and pneumonia, B12 injections, and childhood immunisations.

Practice Manager

The Practice Manager is involved in managing all of the daily business aspects of both practices such as making sure that the right systems are in place to provide a high quality of patient care, human resources, finance, patient safety, premises and equipment and information technology. The Practice Manager supports the GPs and other medical professionals with delivering patient services and also helps to develop extended services to enhance patient care.

The practice secretary deals with all patient referrals between hospitals and GP's, either through Choose & Book or privately, she is able to provide a link between the hospital and the patient in cases of missed appointments, or requests for further information. She is also able to refer patients for a variety of scans through specialised centers.  The secretary also supports the GP's and Practice Manager in administration of the practice.

The dispensary issues all patient repeat prescription medication.  They deal with prescription enquiries and liaise with District Nurses and Community Services regarding medication of housebound and terminally ill patients.  They also provide a dispensing facility for patients over 3 miles away. 

West Horndon is a dispensing practice, patients can pick up their medication directly from the surgery.  They also deal with enquiries and other medical professionals queries.

Asthma is a common long term inflammatory disease of the airways of the lungs

Chronic obstructive pulmonary disease (COPD) is the name for a collection of lung diseases, including chronic bronchitis, emphysema and chronic obstructive airways disease. People with COPD have trouble breathing in and out, due to long-term damage to the lungs, usually because of smoking.

We are offering steroid injections to patients who are suffering for Tendonitis, Bursitis,Rheumatoid Arthritis, Carpal Tunnel, Gout, and Osteoarthritis (less commonly). Please speak to your GP for more information.

The nurse comes in from Basildon for our blood clinic on a Friday at Peartree Surgery on Friday mornings. There are only 20 appointments available so this is done on a first come, first served basis. If you are on Warfarin or fasting please let the reception staff know as you will be given priority. Tickets will not be given out until 9.30am.
