A valid blood test request form will be required, and should include the following information: Full name, Date of birth and NHS number of the patient.

If you feel that your child may benefit from using an anesthetic cream prior to the blood test, this should be discussed with your Doctor or with the person referring you for the blood test. There are simple instructions for use inside the box of the anesthetic cream. Generally , the cream should be applied 1/2 hour before the blood test is performed. If you need advice or assistance with the application, please contact our front desk.
