Asthma Clinic - Asthma Clinic

Appointments for the Asthma Clinic can be booked through the Main Reception on 01889 881206 or call Lisa on 01889 882251.

Baby clinic with health visitor

We have a Health Visitor attached to the Surgery. They are here to help and advise you. They can be contacted on 01889 882386.

Child Immunisations

Chiropody - provided in house and available on NHS

We have a Chiropodist who offers a clinic on a regular basis.

This service requires a referral by your GP

COPD clinic with spirometry

Diabetes Clinic providing insulin initiation

Dressings clinic (nurse led)

This includes leg ulcers and 4 layer compression bandaging.

Long-Acting Reversible Contraception (LARC - eg IUD or implant) - IUDs

Minor surgery (e.g. removal of moles and skin lesions) - provided in-house - Minor Surgery

The Surgery run Minor Surgery Clinics regularly you will need to be assessed by the Doctor and then you will be given an appointment to attend the clinic.

This service requires a referral by your GP

Pharmacy available on-site - Dispensary Service

The Surgery has a Dispensary on site we are able to dispense medications directly to patients living in rural areas more than one mile from the nearest Chemist. If you are eligible, we will ask you to register for this service when you join the Practice.

Phlebotomy

Surgery GP's requesting blood tests can be carried out in the surgery. We also offer Warfarin starts and INR star clinics.

Physiotherapy - provided in house and available on NHS

There is a Physiotherapist that holds a clinic at the Surgery. The Doctor will refer you to the service.

This service requires a referral by your GP

Travel health without yellow fever

The Travel Clinic is a service we offer to our patients. We ask that you complete our travel form that is available from Reception or you can fill in on line at www.hazeldenehousesurgery.org.uk. We ask that you do this at least eight weeks before departure. A member of the Nursing Team will contact you to arrange an appointment.
