Asthma Clinic - Chronic Disease monitoringThe surgery runs a chronic disease monitoring programme for the following diseases:
Diabetes
Asthma
COPD (chronic Lung Disease)
Heart Disease
Kidney Disease
Rheumatoid Arthritis
Patients are invited by letter from the surgery to come to an appointment with one of our Practice Nurses. We will yearly send out letter or phone each of our patients to remind them to book for their chronic disease annual review. It is important each patient maximises our services to their full potential.
The letter or telephone consultation will also tell you if you need to have a blood test in advance of coming for that appointment, and whether you need to bring a urine sample with you.
Child immunisation clinic runs every Friday morning between 09:00am-13:00pm.
Your child will be invited via a letter or telephone call from our administration team. It is important that you bring your child to the allocated appointment as the child will be due immunisations.
If you have any concerns regarding you child immunisations, please feel free to book your child in with the Practice nurse during this clinic.
We currently work alongside the DAAT Team ( Drug and Alcohol Team). We have had a successful attitude towards this clinic in the past year's.
This service can be accessed via a referral from a GP. If you feel you would like to attend this service please ask the GP for more information and how this service can help you.
