Asthma ClinicWe currently see all adult patients with asthma on an annual basis to ensure the condition is under control and the patient stays well. Asthma patients under the age of 19 years old will be asked to see the practice nurse Karen Hall every 6 months or twice per year to ensure the condition is well managerd, a personal asthma action plan will be discussed at the consultation with the nurse and inhaler techniques will be obseved. PLEASE NOTE we are asking that all patients with asthma contact the practice for review around the time of their birthday.
Child health and development - Child Health Surveillance (CHS)Invites will be sent out from the local Child Health dept and will be scheduled in line with the national immunisation guidelines for children and babies. If you have any questions regarding your childs immunisations please contact the practice.
COPD clinic with spirometry - and Other Disease Register Reviews:Patients with any of the following conditions will be asked by the practice to attend an annual review with one of our trained healthcare professionals. The reviews recalls will be around your birth month as this makes it easier to remember and patients can ring the practice to book an appointment. Diabetes, COPD, Asthma, Heart Disease, Kidney Disease, Hypertension, Stroke or Mental Health.
Learning disability health checkAll patients who are registered with the practice will be called each year to have a full health check. Patients will be asked some questions about any health issues or problems in order for the practice to ensure the correct care pathway is followed. All patient are reassured and are welcome to be accompanied by a carer, relative or friend.
